Department: Holy Name Medical Center
Position Summary: As a Palliative Care Coordinator, your primary responsibility is to manage and facilitate referrals to Supportive Care services, including Hospice and Palliative Care, in accordance with established admission criteria. You will recommend alternative care plans as necessary and enhance access to Hospice services. This role involves acting as an on-site resource for the Hospice team, assisting facility staff in referring patients to Hospice Services. The Palliative Care Coordinator will cultivate and sustain relationships with both internal and external referral sources, assess patient eligibility for transfer, educate patients and their families about hospice care options, and serve as a liaison between the Hospice Team and referring providers or facilities. Travel may be required to evaluate referrals and foster professional relationships.
Requirements:
Education:
- Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW) preferred.
Skills:
- Comprehensive understanding of hospice criteria and services.
- Ability to work independently while effectively communicating with the healthcare team.
- Exceptional communication, negotiation, and public relations skills.
- Proficient in marketing to referral sources and the community.
- Strong counseling abilities to provide education and support to terminal patients and their families.
- Flexible and cooperative in fulfilling all responsibilities. Must possess a valid driver's license and reliable transportation.
Experience:
- A minimum of two years of direct patient care experience in a hospital or similar healthcare environment is preferred.
Certification/License:
- Registered Nurse or Social Worker license in the State of New Jersey is preferred.
Essential Functions of the Job:
- Collects and documents referral information, including current medications and treatments, and ensures agreement from the physician, patient, and primary care person for Hospice Services.
- Collaborates with the Hospice Medical Director and Interdisciplinary Team to evaluate patient referrals and determine suitability for admission to the Hospice Program or other Supportive Care services.
- Provides consultation and guidance to referrers, patients, and families regarding hospice care and end-of-life options.
- Coordinates with the Hospice Team to ensure that necessary equipment and services are arranged prior to patient admission for hospice care.
- Informs the Hospice Team of patient acceptance for services and secures signed hospice consents, providing essential medical background and preliminary care plans necessary for admission.
- Assists case managers in ensuring timely referrals to hospice services.
- Works with the Intake Team on insurance coverage for requested hospice services and educates patients and families about coverage options.
- Establishes and nurtures collaborative relationships with referral sources, institutional personnel, and other facilities to ensure appropriate referrals.
- Acts as a liaison for patients and families during the transition from discharge to the initiation of Hospice Program services.
- Participates in Interdisciplinary Team meetings to exchange information on patient care planning.
- Engages in Quality Improvement initiatives aimed at enhancing patient care quality.
- Contributes to special projects and performs additional duties as assigned by Administration.
- Maintains familiarity with the Medicare Hospice Benefit and educates patients, families, and healthcare professionals accordingly.
- Generates appropriate referrals to Hospice, Palliative, and Holy Name Medical Center from existing and new referral sources.
- Establishes and maintains professional relationships with all referral sources, including physicians, nursing homes, assisted living facilities, and hospital personnel.
- Upholds high standards of customer service and demonstrates respect for all stakeholders, both internal and external.
- Participates in evening and weekend calls for admissions as required, responding competently and promptly.
- Performs additional responsibilities as needed to support various team functions.
