LPN Nurse Coordinator

Promesa R.H.C.F. is seeking a highly skilled LPN Nurse Coordinator to join our team. As a key member of our nursing staff, you will be responsible for coordinating and supervising the implementation of nursing care for our residents.
Key Responsibilities- Provide Direct Care: Follow accepted standards of nursing practice in providing direct care, performing procedures, and using equipment.
- Assess and Coordinate Care: Assess clients at the time of intake, and then on an ongoing basis. Coordinate all necessary client services with other interdisciplinary team members in the comprehensive care planning process.
- Administer Medication and Treatment: Administer STAT medication and inoculations and render treatment as prescribed by the Primary Care Provider.
- Maintain Accurate Documentation: Maintain accurate documentation of all nursing activities.
- Participate in Quality Improvement: Participate in the formulation and evaluation of effectiveness of nursing standards within the department and participate in appropriate Continuous Quality Improvement activities.
- Education and Experience: High School Diploma or GED in an equivalent related field required. Completion of LPN Education Program. New York State Licensed Practical Nurse licensure required. Experience working in a nursing home and/or addiction setting preferred.
- Skills and Abilities: Ability to use business acumen to exercise good judgment and decision-making. Ability to deal with sensitive issues with diplomacy and discretion. Must be able to multi-task with strong organizational skills. Excellent interpersonal skills and able to communicate both verbally and in written form. Commitment to cultural diversity and sensitivity. Ability to maintain a professional demeanor under pressure, and operate with a keen sense of urgency and commitment to quality.
