Social Services Coordinator

- Program Development: Collaborate with the Social Service Director to design and execute social service programs tailored to the needs of residents, ensuring alignment with facility goals and regulations.
- Collaboration: Work closely with administration, medical, nursing, and other departments to strategize and enhance social services, fostering a culture of teamwork and resident-centered care.
- Policy Management: Assist in developing, implementing, and reviewing department policies and procedures, ensuring compliance with regulations and best practices, and promoting a culture of accountability and transparency.
- Resident Support: Guide residents in achieving self-care, independence, and well-being through tailored social services, including counseling, education, and advocacy.
- Community Engagement: Participate in community planning to align facility services with resident and family needs, promoting partnerships and collaborations that enhance the quality of life for our residents.
- Documentation: Maintain accurate and comprehensive progress notes and records, tracking residents' responses to treatment plans and ensuring compliance with regulatory requirements.
- Resource Coordination: Connect residents with community resources and assist in scheduling necessary outpatient appointments, ensuring seamless transitions and optimal outcomes.
- Discharge Planning: Collaborate with agencies and individuals to facilitate seamless discharge processes for residents, ensuring a smooth transition to community-based care.
- Training and Development: Contribute to training programs focused on the social and emotional needs of residents, promoting a culture of learning and professional growth.
- Budget Assistance: Assist in the preparation and management of the department's budget, ensuring effective resource allocation and fiscal responsibility.
- Compliance: Report any suspected fraud or unauthorized disclosures of resident information, ensuring confidentiality is maintained and regulatory requirements are met.
- Bachelor's degree in social sciences or related field.
- Licensed Social Worker as such by the California Board of Behavioral Science Examiners.
- Strong written and verbal communication abilities.
- Proficient in interpreting and analyzing information.
- Ability to solve practical problems in diverse situations.
- Familiarity with healthcare regulations and social services.
- Ability to lift and/or move up to 25 pounds occasionally.
- Frequent use of office equipment (computer, copier, etc.).
- May require assistance in emergency situations.
- The work environment is generally low to moderate noise, conducive to focused work.
