About the JobThe Nurse Navigator will be responsible for:
- Providing direct and indirect care to patients and significant others under the direction of the Physician and Program Director
- Consulting on case management issues in areas of targeted intervention
- Serving as a care coordinator ensuring timely access and seamless coordination of services
To be considered for this rewarding position, you must have:
- A minimum of 5 years' experience working as a RN in an acute care hospital setting
- A current and valid state RN License
- OCN certification preferred
- A current BLS certificate upon hire and maintain current
- Nursing experience in related fields (Breast, GYN) preferred
