Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will provide high-quality nursing care to patients in post-acute facilities.

About the Job

  • Monitor patients' physical and mental conditions, track vital signs, and document medical histories and health concerns.
  • Administer medications, track their use, and monitor effects, while maintaining accurate records.
  • Cultivate a comfortable, safe facility for residents by adhering to infection control protocols and performing routine cleaning tasks.
  • Collaborate with staff to develop and implement individualized care plans that address patients' unique needs.

Requirements

  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Minimum of 1 year of experience in a related role or completion of an approved LPN educational program
  • High School diploma or equivalent required; graduation from an approved LPN educational training program preferred
