We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Peoples Health. As a CNA, you will play a vital role in providing direct patient care services and assisting patients and nursing staff in implementing nursing measures as planned by the Registered Nurse.
Key Responsibilities- Participate in patient care conferences and meetings to ensure effective communication and coordination of care.
- Attend continuing education programs to stay up-to-date on the latest developments in hospice and home health care.
- Provide personal hygiene services, including shower, tub, or bed baths, special skin care, mouth care, hair care, and changing bed linens, as directed by the RN.
- Assist patients with transfers, ambulation, and mobility, including the use of mechanical lifting equipment when necessary.
- Assist patients with walking, including the use of walkers and wheelchairs, when applicable.
- Assist with meal preparation and feeding, when required.
- Perform incidental household services essential to the patient's health care at home and necessary to prevent or postpone institutionalization.
- Assist patients with prescribed exercises when instructed by the appropriate health professional.
- Measure and record fluid intake and output, as directed, and take and record blood pressure, pulse, and respirations.
- Complete a worksheet for each day summarizing care provided to each patient.
- Record and report changes in the patient's physical condition, behavior, or appearance immediately to the nurse supervisor.
- At least one year of experience in hospice or home health care and an adequate understanding of the elementary concepts of hospice or home health care and your role as a member of a patient care team.
- Holds the appropriate certificate and/or has completed the required curriculum/hours and competency has been evaluated in accordance with Federal/State regulations.
- Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, family, and other personnel.
