Job Overview

Position Summary

The primary responsibility of this role is to oversee nursing staff, monitor patient conditions, implement treatment plans, and develop comprehensive care strategies for long-term patient needs. This position involves administering medications, engaging with families, and ensuring the highest standard of care for all residents/patients. Compliance with current federal and state regulations, as well as company policies, is essential.

Key Responsibilities

  • Deliver exceptional patient care to all facility residents.
  • Accurately document care provided and observations of resident conditions in the clinical record, adhering to standard clinical documentation protocols as mandated by the facility and applicable regulations.
  • Maintain communication with the resident's healthcare provider.
  • Complete all required nursing documentation, including assessments, orders, progress notes, and any assigned tasks, ensuring accuracy in reflecting the services rendered to residents.
  • Participate in medication management to ensure safety and effectiveness for residents by:
    • Administering prescribed medications, including the next scheduled dose upon admission or readmission, and notifying supervisors of any unavailable medications;
    • Preparing, administering, and documenting medications as directed;
    • Monitoring and reporting resident responses to medications;
    • Identifying and promptly communicating any adverse drug reactions;
    • Initiating enteral or parenteral therapy as required;
    • Maintaining accurate narcotic records;
    • Coordinating the ordering of pharmaceuticals;
    • Informing physicians of automatic stop orders;
    • Completing reviews of recapitulated physician orders as assigned.
  • Assist with or implement emergency measures for sudden adverse developments in residents, including directing and administering cardiopulmonary resuscitation.
  • Exhibit punctuality and regular attendance for scheduled shifts.

Additional Responsibilities

  • Ensure adherence to the company's clinical policies and procedures.
  • Follow guidelines established by the Nurse Practice Act, company standards, and sound nursing judgment.
  • Perform other related duties as assigned.

Knowledge, Skills, and Abilities

  • Maintain confidentiality and protect resident rights regarding all personnel and resident information.
  • Possess a strong understanding of HIPAA laws and related regulations.
  • Demonstrate strong communication, customer service, teamwork, and organizational skills.
  • Exhibit a positive and enthusiastic attitude.

Qualifications

Education

Graduate of an accredited School of Nursing; BSN preferred.

Licenses/Certification

Current RN/LPN Licensure in the state of employment.

Valid CPR Certification.

Experience

Preferred six months of experience in a long-term care setting.
