Salary: $110,000 - $140,000 per year

Job Summary:

We are seeking a skilled Nurse Practitioner or Physician Assistant to lead the medical aspects of team-based care. In this role, you will conduct whole-person assessments, collaborate closely with community primary care and behavioral health providers, and provide direct patient care for unscheduled needs.

Responsibilities:

  • Clinical Leadership: Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health priorities.
  • Care Coordination: Support care coordination and lead care and disease management, addressing care gaps.
  • Community Partnerships: Coordinate with care providers at CMHCs, FQHCs, and hospitals.

Qualifications:

  • Nurse Practitioner (NP) and/or Physician Assistant (PA) license in Ohio
  • 2+ years of experience as a Nurse Practitioner or Physician Assistant
  • Experience working with behavioral health patients
