Key Responsibilities
  • Assess and implement individualized care strategies for members, focusing on the most appropriate and least restrictive level of care.
  • Identify and initiate referrals for member transition to community resources and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage the care plan throughout the continuum of care as a single point of contact.
  • Communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for members and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

Requirements
  • Current, unrestricted RN license in the state of Texas.
  • 2+ years of experience working within the community health setting or in a healthcare role.
  • Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications.
  • Reliable transportation and ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
  • Reside in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service.
