Job Summary

CAMBA is seeking a skilled Case Manager to join our team at Apex Place Supportive Housing in Forest Hills, Queens. As a Case Manager, you will work closely with tenants to provide individualized support and guidance to help them achieve their goals and maintain stability in their lives.

Key Responsibilities
  • Conduct initial intake and assessment of tenants and their families' needs and periodic re-assessments.
  • Develop and implement independent living plans, including short-term and long-term goals, in collaboration with tenants.
  • Assist tenants in accessing community resources and making referrals to appropriate services.
  • Monitor tenants' progress and document via progress notes.
  • Participate in case conferences and follow-up with tenants and referral organizations.
Requirements
  • Bachelor's degree (e.g., B.A., B.S.W.) and experience working with the seriously and persistently Mentally Ill (SPMI) population.
  • Must sign NYC DOH/MH attestation stating that candidate has not been the subject of a founded child abuse report.
