Case Manager - Bedford Hills

Hour Children is seeking a skilled Case Manager to join our team at Bedford Hills. As a Case Manager, you will provide comprehensive case management services to mothers and single women transitioning back into the community from incarceration.

Key Responsibilities:
  • Assess treatment needs and develop, monitor, and evaluate treatment plans and progress.
  • Facilitate interdisciplinary approaches and monitor staff performance.
  • Admit new clients by reviewing records and applications, conducting orientations, and working closely with counselors and other staff.
  • Determine clients' requirements by completing intake interviews, reviewing therapist evaluations, and identifying need for therapeutic medical, psycho-social, and psychiatric evaluations.
  • Monitor cases by verifying clients' attendance, observing and evaluating treatments and responses, advocating for needed services and entitlements, and intervening in crises.
  • Maintain clients' records by reviewing case notes, logging events and progress, and communicating clients' progress to the interdisciplinary team and family.
  • Work with counselors to carry out clients' discharge by reviewing and amplifying discharge plans, coordinating discharge and post-discharge requirements, and orienting and training family members.
  • Provide resources to clients, families, and others as needed.
  • Improve treatment results by studying, evaluating, and redesigning processes, implementing changes, and rewriting policies and procedures.
  • Prepare reports by collecting, analyzing, and summarizing treatment and results data and trends, compiling statistics, and completing grant and subsidy applications.
Requirements:
  • BA in social work or other relevant degree with 3 years' experience or 5+ years case management experience, preferably in human services with equivalent combination of training and experience.
  • Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.
  • Bilingual in English and Spanish a plus (read, write, and speak proficiently).
  • Previous experience working with adult community and reentry programming preferred.
  • Must have a valid in-state driver's license.
  • Experience with data entry and proficiency with computers, including Microsoft Office Suite and electronic data base systems.
  • Ability to work independently, be flexible and dependable, and demonstrate initiative and creativity.
Physical Requirements:
  • Ability to walk around the facility and carry 20 or less pounds.
