Qualifications:
- Graduation from an accredited Nurse Practitioner or Physician Assistant program.
- Valid state licensure and certification as a Nurse Practitioner or Physician Assistant.
- Demonstrated clinical experience as an Advanced Practice Provider, ideally in a primary care or specialty environment.
- Strong clinical and diagnostic capabilities with the ability to formulate effective treatment strategies.
- Exceptional communication and interpersonal skills for collaboration with patients, families, and the healthcare team.
- Able to excel in a dynamic environment while managing multiple tasks.
- Commitment to delivering patient-centered care with a focus on compassion, empathy, and respect.
- Dedication to ongoing professional growth and keeping up with the latest medical advancements.
- Manage an average daily rehabilitation census of 25 patients.
- Effective time management and proactive communication are essential for success.
- Conduct thorough patient evaluations, including medical history assessments, physical examinations, and ordering diagnostic tests.
- Diagnose and treat common acute and chronic conditions within the scope of practice.
- Design and implement individualized treatment plans, including prescribing medications, therapies, and lifestyle adjustments.
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care and continuity.
- Educate patients and their families on disease management, preventive care, and healthy lifestyle choices.
