We are seeking a skilled Nurse Practitioner to join our HouseCalls team in Florida and the US Virgin Islands. As a HouseCalls Nurse Practitioner, you will conduct in-home assessments, perform evidence-based practice screenings, and identify diagnoses to inform care management.
Key Responsibilities:
- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams
- Perform point-of-care testing and evidence-based practice screenings as appropriate
- Identify diagnoses and formulate a list of current and past medical conditions using clinical knowledge and judgment
- Communicate findings to inform member's primary care physician of potential gaps in care
- Intervene in urgent and emergent situations as needed
- Educate members on disease processes, medication, and compliance
Requirements:
- Current licensure in Florida and the US Virgin Islands
- Active NP license or ability to obtain by start date
- APCs must have obtained approval to practice autonomously or without formal supervision from their licensing board, if applicable
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
- Access to reliable transportation for travel up to 85% within licensed states
Preferred Qualifications:
- 1+ years of clinical experience in their highest level of education, clinical setting
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
- Home health care or home visit experience
