We serve children, transitional age youth, families, adults, and older adults, offering a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education, and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare, and Ventura Counties.

Job Summary

This role works within the vision, mission, and philosophy of Pacific Clinics to provide support and coaching on specific skill sets for caregivers, as well as to provide a parent perspective in various settings. The Family Partner I will support the development of a shared understanding between individuals and groups based on the twelve principles of the agency service philosophy.

Responsibilities
  1. Utilize personal and professional life experience to provide peer support to parents and caregivers served by the program as appropriate.
  2. Provide parents and caregivers with information about Pacific Clinics and the process and procedures, including the Family Partner I role to assist them.
  3. Participate in providing a variety of support services for parents/caregivers, such as family events, family activities, and recognition events.
  4. Coach families to make decisions and develop plans based on their needs, and to use their voice to direct treatment.
  5. Collaborate and participate in Child and Family team (CFT) meetings to ensure the parent and family members have access, voice, and choice within the process and to support the parent/caregiver's connection to the CFT members, as necessary.
  6. Identify community connections for internal teams.
  7. Serve as a consultant and support shared understanding to ensure that family and providers understand each other's perspective and information.
  8. Provide non-judgmental, culturally informed support to parents and caregivers.
  9. Explore, identify, and work to (re)establish natural support systems as needed; identify and link family with community connections and resources.
  10. Model and coach effective coping techniques for parents and caregivers.
  11. Provide a caregiver's perspective in agency service support structures.
  12. Assist with the design and implementation of internal and external trainings about the parent perspective.
  13. Utilize conflict resolution skills.
  14. Participate in continuous learning processes as it relates to engaging youth and families.
  15. Maintain and manage schedules effectively to manage the needs of multiple families, meetings, external, and internal customers.
  16. Produce and maintain accurate and timely documentation within agency standards.
  17. Hold staff accountable to service delivery principles related to the family voice, choice, access, pace, and process, through sharing personal experience, engaging in team meetings, and consultations with staff internally and externally.
  18. Participate in service improvement and development efforts.
  19. Participate in program and system meetings.
  20. Perform other responsibilities, as assigned, to support specific department/business needs.

MINIMUM EDUCATION AND/OR EXPERIENCE:

  • High School diploma or GED required.
  • Be able to articulate the experience and perspective of a parent/caregiver or relative of a child with complex needs involving mental health, child protective, or probation services. This ability is typically acquired as the primary caregiver of a child with complex needs.
  • 3 years as a parent or caregiver of a special needs child.

OTHER SPECIFIC REQUIREMENTS:

  • Therapeutic crisis intervention and Agency training.

PREFERRED QUALIFICATIONS:

  • Some college education is preferred.

HIGHLY DESIRABLE QUALIFICATIONS:

  • Speak, read, and/or write another language.
