Job Title: Clinical Care Specialist

Hire Velocity is a skilled home health care agency providing high-quality nursing and therapy services in central Indiana. Our experienced clinicians are dedicated to delivering exceptional patient care.

We prioritize improving patients' health and independence while they recover at home.

Key Responsibilities:
  • Conduct regular home visits to assess patients' needs and progress.
  • Develop and implement personalized care plans under physician guidance.
  • Evaluate patient reactions, monitor progress, and document symptoms.
  • Provide education on disease management, medications, and treatment plans for patients and caregivers.
  • Counsel patients and caregivers on in-home care techniques.
  • Coordinate patient services and supervise LPNs and HHA's.
  • Notify physicians of any changes in patient condition.
  • Ensure compliance with agency policies and procedures.
Qualifications:
  • Nursing degree from an accredited institution.
  • Licensed to practice nursing in the state where the agency operates.
  • At least one year of RN experience.
  • Strong communication and interpersonal skills.

Estimated salary range: 60000-90000 per year.
