h3Job Description/h3Memory Care Nurse Role at Three Pillars Senior Living CommunitiespWe are seeking a compassionate and skilled RN or LPN Nurse to join our team at Riverside Lodge, a brand-new assisted living community in Dousman, WI./ppThe successful candidate will provide holistic care for residents based on individual care plans, supporting our mission and values of Respect, Commitment, Fulfillment, Empathy, and Fun./ppbKey Responsibilities:/b/pulliSafely administer medications and treatments as ordered by the physician and in accordance with accepted standards of practice and Three Pillars policies and procedures./liliMaintain timely, accurate written and electronic charts/documents/records as required by law, regulations, and Three Pillars policies and procedures./liliObserve and record resident condition information, reporting findings to and collaborating with RN to develop and implement a plan of care for the resident./liliCommunicate resident change of condition to appropriate persons, including nurse manager/supervisor, RN, physician, family, clergy, hospice team, and interdisciplinary team./liliAssist in the admission/readmission process as directed by the RN, including observing and gathering clinical data and developing and implementing the initial plan of care./liliObtain, transcribe, and implement physician's orders according to accepted standards of practice and Three Pillars policies and procedures./liliSupport and participate in all resident care programs, including following individual POCs, documentation, communication of observations, and offering feedback/suggestions./li/ulpbRequirements:/b/pulliMinimum education, certification, and/or licensure: Licensed Practical Nurse in Wisconsin./liliCurrent and valid CPR certification./liliExperience in long-term care preferred./liliBasic computer knowledge./liliAble to frequently push/pull objects weighing 60 pounds and occasionally lift and move objects weighing 100 pounds or more./li/ulpThree Pillars Senior Living Communities offers a fulfilling career where you make a difference in people's lives every day.
