Position Overview:


This role involves supporting physicians and/or registered nurses in delivering clinical care and overseeing the care provided by medical assistants. The position requires collaboration with physicians and other healthcare professionals to address the needs of patients and their families.

The practical nurse will execute the care plan by offering direct basic nursing services in line with the LPN scope of practice as outlined in the Nurse Practice Act. Additionally, the nurse will participate in evaluating patient responses to nursing interventions and will document these observations accurately and promptly.


Qualifications:
Completion of an accredited technical, professional, or vocational nursing program
Basic Life Support certification for Healthcare Providers (BLS)
Licensed Practical Nurse credential
Current RN licensure is also acceptable
Ability to function effectively within a multidisciplinary team
1-2 years of experience in a relevant field

Key Responsibilities:
Administers medications to participants safely and in accordance with legal and professional standards. Accurately interprets and executes physician orders, documenting medication administration in a timely manner. Provides education to participants and caregivers regarding medications and addresses any inquiries.
Performs technical nursing skills in compliance with hospital and departmental policies, procedures, and accepted standards of practice.
Assesses supply needs for the nursing department and orders necessary supplies to ensure departmental needs are met.

Organizational Responsibilities:

Identifies personal areas for improvement, collaborates with clinical instructors to enhance learning experiences for nursing students, and stays updated with current practices in the field. Participates in the monthly review of Medication Administration Records (MAR) and develops MAR for new participants as needed.
Collects data contributing to nursing assessments, documents ongoing observations, and communicates findings to other healthcare team members. Demonstrates knowledge of PACE regulations, particularly those related to primary care provision for participants.



Additional Information:

  • Delivers direct patient care tailored to individual patient needs.
  • Plans and implements care based on daily patient assessments, Individual Education Plans (IEP), Individualized Health Plans (IHP), and/or physician directives.

  • Encourages optimal patient wellness by assisting patients in adapting to their health conditions.

  • Reports significant changes in patient status to the appropriate personnel.
  • Maintains clinical records as required.
  • Enhances professional growth and commitment to providing exceptional service to patients, families, administration, and colleagues.

  • Performs tasks as delegated by the RN.
  • Assists with various duties as needed, including providing medical assistance for students, collaborating with peers in the classroom, supporting assessments, and managing immunization records and screenings.

  • Maintains professional competency and pursues ongoing development through continuing education and participation in relevant conferences and seminars.
