About the Role

This is a unique opportunity for a Licensed Practical Nurse (LPN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a Home Health Nurse, you will have the flexibility to work on a part-time basis, with shifts available in the Crossville/Monterey area.

Key Responsibilities
  • Verify the patient's plan of care with the Registered Nurse (RN) prior to each visit.
  • Collaborate with the physician and RN to ensure the patient's needs are being met, and notify them of any changes in the patient's condition.
  • Teach patients and caregivers self-care techniques, and review all medications and perform a medication reconciliation at each visit.
  • Perform specialized procedures ordered by the physician, such as foley catheter changes.
  • Prepare equipment and materials for treatments using aseptic technique, and complete documentation timely and accurately.
Requirements
  • One year of clinical LPN experience.
  • Current LPN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
