Job Title: Advanced Practice Provider

We are seeking a highly skilled Advanced Practice Provider to join our team at i-Health Inc. As an Advanced Practice Provider, you will be responsible for providing primary care services to our patients, including assessing, diagnosing, prescribing, treating, and educating patients.

Key Responsibilities:
  • Assess patient status by obtaining health history through patient/family interviews and chart reviews.
  • Develop and implement treatment plans by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.
  • Handle minor procedures and instruct patients regarding medications and treatments.
  • Communicate appropriate case management information to other professionals and community agencies.
  • Prepare documentation for medical records, including updating patient medical charts.
Requirements:
  • Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner or physician assistant program.
  • Current CPR certification and graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • NCCPA certification and Minnesota Board of Medical Practice registration/license.
  • DEA registration and experience in family/internal medicine preferred.
