About the Role

As a Hospice RN at Southcoast Health System, you'll be responsible for providing high-quality, patient-centered care to individuals with serious illnesses. You'll work closely with a multidisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible care and support.

Responsibilities
  • Assess and develop individualized care plans for patients and families
  • Provide direct care to patients, including medication management and wound care
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Document patient progress and outcomes
  • Participate in quality improvement initiatives to enhance patient care
Requirements
  • Current RN licensure in the Commonwealth of MA and RI
  • 2+ years of hospital, medical/surgical, or other relevant nursing experience
  • Hospice and Palliative Care Certification preferred
  • Basic life support (BLS) certification required
