Key Responsibilities:
• Attend to personal care needs as assigned and respond promptly to patient requests.
• Communicate any inability to perform tasks to the Case Manager immediately.
• Utilize positive communication techniques while respecting patient confidentiality and ethical standards.
• Ensure patient safety by using equipment correctly (e.g., foot stools, side rails, O2).
• Provide personal care services, including bathing, back rubs, oral hygiene, and changing bed linens as needed.
• Assist patients with dressing and undressing as required.
• Plan and prepare nutritious meals, including shopping, as assigned.
• Aid in feeding patients as necessary.
• Record vital signs such as temperature, pulse, respiration, and blood pressure as per care plans.
• Monitor and care for patients' skin to prevent tissue breakdown.
• Report significant changes in patient condition to the Case Manager.
• Be aware of the dynamics involving caregivers and other individuals in the patient's environment.
• Assist with ambulation and exercises as directed by healthcare professionals.
• Perform range of motion exercises and assist with patient positioning as instructed.
• Support patients with self-administered medications as permitted by state regulations.
• Provide assistance with light laundry tasks as assigned.
• Carry out simple procedures as part of therapy services when required.
• Offer respite care for patients and their families when on-site.
• Maintain cleanliness and orderliness in the patient's living area as assigned.
• Follow organizational documentation and care standards, adhering to professional conduct.
• Participate in quality assessment and performance improvement initiatives.
• Show respect for the patient and family environment, addressing personal needs appropriately.

Qualifications:
• Completion of state training requirements.
• At least 75 hours of classroom and supervised practical training, with 16 hours of classroom training completed before practical training.
• Minimum of six months of experience in providing personal care.
• Must be at least 18 years old.
• Ability to read, follow written instructions, and document care provided.
• Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
• Positive references from previous employers.
• Ability to work independently with minimal supervision.
• Comfort with issues surrounding death and dying.
• Maintain a calm demeanor in patients' homes.
• Current CPR certification is required.
• Exhibit tact, patience, and good personal hygiene.
• Successfully complete a probationary period to demonstrate competency in required qualifications.
• Strong oral and written communication skills.
• Good organizational abilities.
