About the Role

Tufts Medicine Care at Home is seeking a skilled RN Case Manager to join our team. As a key member of our healthcare team, you will be responsible for delivering comprehensive home health services to clients in their place of residence.

Key Responsibilities
  • Assess patients' physical and psychosocial needs in a sensitive and caring manner, following established standards of nursing practice and VNA procedures.
  • Establish realistic goals and develop plans of treatment in cooperation with the patient, family, and members of the healthcare team.
  • Collaborate with patient/family and other healthcare providers and/or community resources to plan care and discharge.
  • Complete physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies.
  • Effectively manage assigned caseload, within the team model of care delivery.
