We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a key member of our pharmacy team, you will play a critical role in providing exceptional patient care and support to our patients in long-term care settings.
Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure laboratory tests are ordered, evaluated, and discussed with the healthcare team as necessary.
- Collaborate with the healthcare team to provide clinical and technical support.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Implement policies and procedures for safe drug procurement, storage, distribution, and use.
- Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
