LHC Group is seeking a skilled Occupational Therapist to join our Home Health team in Belpre, Ohio.
Job SummaryWe are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes.
Responsibilities- Conduct thorough assessments and evaluations of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
- Develop and implement individualized therapy plans in accordance with the plan of care, using evidence-based techniques and in coordination with other members of the healthcare team.
- Evaluate patient progress and make adjustments to the therapy plan as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Current Occupational Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
