Job Overview:

The Texas Department of Aging & Disability Services is seeking a dedicated professional to join our Program Support Unit (PSU) as a Community Care Worker III. This role involves executing moderately complex responsibilities associated with managed care Long Term Services and Supports (LTSS).

The position requires regular interaction with various stakeholders, including case managers, individuals seeking waiver services, service providers, and other relevant entities. The successful candidate will operate under general supervision, exercising initiative and independent judgment while utilizing a computer and demonstrating extensive knowledge of CCSE/PSU programs and associated software.


Key Responsibilities:

- Engage in case management activities, assisting individuals in selecting managed care organizations (MCOs) and determining eligibility for Medicaid waiver programs.

- Facilitate the transition of individuals from nursing facilities to community settings, ensuring a smooth process for those aging out of the Medically Dependent Children Program (MDCP).

- Maintain accurate waiver eligibility records and communicate eligibility determinations to members and key stakeholders.

- Conduct interest list activities, confirming contact information, mailing enrollment packets, and following up as necessary.

- Interpret policies and coordinate activities related to transitioning individuals between care settings.

- Address continuum of care issues for Medicaid waiver members experiencing service gaps.

- Assist members denied services in filing fair hearing requests and monitor the appeal process.


Required Knowledge, Skills, and Abilities:

- Comprehensive understanding of Medicaid waiver programs, policies, and procedures.

- Familiarity with community and government service delivery systems, regulations, and interviewing techniques.

- Proficient communication skills, both oral and written, to effectively interact with diverse individuals.

- Strong organizational skills to manage multiple tasks and meet program objectives efficiently.

- Ability to identify potential issues affecting service delivery and proactively address them.

- Competence in using computer software, including Outlook, Excel, and Word, along with an understanding of state software programs.
