Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Home Risk Assessment Specialist, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.

Responsibilities include:

  • Conducting comprehensive health assessments in a variety of venues, including home visits, skilled facility visits, and telehealth visits
  • Collaborating with primary care physicians on patient education and follow-up
  • Providing services in a flexible and dynamic environment
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Nursing or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A current BLS, ACLS, or CPR certification
  • A minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with employee health/medical record software
  • Excellent verbal and written communication skills

Travel requirements include the ability to travel, a valid state driver's license, and adequate automobile insurance coverage.
