Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at SELF Inc. as a Client Services Case Manager. In this role, you will provide trauma-informed case management support to participants in various emergency housing, Safe Haven, and supportive housing programs.

Key Responsibilities
  • Provide case management services to a caseload of 25-35 participants, conducting initial assessments and developing individualized service plans to connect participants with necessary resources.
  • Identify and connect participants with alcohol, drug, mental health, or medical services, arranging evaluations and scheduling appointments as needed.
  • Maintain accurate records of participant progress, health, and welfare, ensuring compliance with data management systems and reporting requirements.
  • Develop and maintain participant case records, compile homeless history, and document case management services, interactions, and participant progress.
  • Participate in collaborative meetings with external agencies, providing information and supporting participant progress.
  • Meet all requirements for submitting required reports to Supervisor, including weekly work schedules, monthly services reports, and housing interventions.
Requirements
  • Bachelor's Degree in a human services field, including Social Work from an accredited University or College.
  • Minimum of three years of acceptable training and experience in homeless, mental health, and/or addiction treatment services.
  • Experience in direct service and/or advocacy is required, with a strong preference for experience in a residential setting.
  • Must possess strong verbal and written communication skills, with the ability to assess participant mental status, monitor, record, and develop individual treatment plans and goals.
