NHCH Caregiver

As a Caregiver for Nuvance Health at Home, you will provide assistance to clients with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as directed on clients' individual care plans. This role requires a strong understanding of the certified nursing assistant (CNA) scope of practice and adherence to NHCH policies and procedures.
Responsibilities- Assist clients with personal care activities, including bathing, oral hygiene, feeding, and dressing.
- Assist clients with exercises, ambulation, and transfer activities, including the use of devices such as canes, walkers, crutches, and lifts.
- Independently assist clients with meal preparation and cooking, and/or feeding, while maintaining sanitary kitchen conditions to prevent food-borne illnesses.
- Assist clients with medication prompts to remind them to self-administer their medications.
- Identify and report changes in client condition.
- Provide basic homemaking and laundry assistance for clients in their homes.
- Follow policy and procedure for assigned client care activities.
- Maintain a safe and orderly work environment in clients' homes, including maintaining professional boundaries and integrity of client financial and personal information.
- Provide companionship and engage in activities of interest to clients.
- Assist clients with light pet care as needed, such as walking a dog, feeding a dog/cat, changing a cat litter pan.
- Transport clients to appointments and errands.
- Fulfill all compliance responsibilities related to the position.
- Perform other duties as assigned.
- Graduate of a nurse aide, home health aide, or patient care tech program, or a minimum of 5 years' experience working as an aide in home care.
- Valid Driver's License, Satisfactory Driving Record (Meets requirements of NHCH policy).
- Ability to read, write, speak, and comprehend the English Language.
- Must be willing to travel 40 or more miles to cover shifts.
- Certified Nursing Assistant.
- HS Graduate or Equivalent.
