Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Port Carbon, Pennsylvania.
Job Summary:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work independently to assess and develop individualized care plans, implement therapeutic interventions, and educate clients and their families on rehabilitative care and activities.
Responsibilities:- Conduct home visits to assess clients' physical needs and develop personalized care plans.
- Implement manual therapeutic exercises, gait training, balance, and other interventions to promote clients' health and safety.
- Educate clients and their families on rehabilitative care and activities to support independent living.
- Document observations, interventions, and evaluations in a state-of-the-art touch pad tablet.
- A current Pennsylvania Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
