Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in River Edge, New Jersey.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in their homes or alternate community settings.

Responsibilities:
  • Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
  • Develop and implement personalized plans of action in collaboration with physicians to ensure optimal patient outcomes.
  • Document all changes in patient condition and update care plans accordingly.
  • Identify counseling needs and plan appropriate teaching strategies to promote patient education and empowerment.
  • Monitor the quality and appropriateness of services provided to ensure the highest level of care.
  • Adhere to agency infection control and safety policies to maintain a safe and healthy environment for patients and staff.
  • Meet the organization's policy on absenteeism and appearance to ensure a professional and respectful work environment.
Qualifications:
  • Physical Therapy Licensure in NJ is required.
  • 2-3 years of prior clinical experience and community-based experience are preferred.
  • Strong computer skills are necessary for documentation and communication.
  • Excellent communication and interpersonal skills are essential for effective patient care and collaboration with healthcare teams.
  • A valid NJ driver's license is required for travel to patient locations.
Working Conditions/Physical Demands:

This role requires working indoors and outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, and use of phone and tablet for documentation.
