Job SummaryWe are seeking a compassionate and skilled Registered Nurse to join our team. As a Nurse Care Manager, you will be responsible for creating a warm and friendly environment for our residents, advocating for their needs, and developing personalized care plans to ensure their comfort and well-being.
Key Responsibilities- Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
- Provide high-quality nursing care, including medication administration, wound care, and vital sign monitoring.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and resident satisfaction.
- Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making.
- Participate in quality improvement initiatives to enhance resident outcomes and satisfaction.
- Current RN licensure in the state of Indiana.
- Minimum 1 year of experience in long-term care or a related field.
- Excellent communication and interpersonal skills.
