Patient Care Coordinator

We are seeking a skilled Patient Care Coordinator to join our Inpatient Nursing team at Loma Linda University Medical Center.
This role requires an individual with excellent clinical knowledge and leadership skills, able to coordinate and document patient care in a fast-paced environment. As a Patient Care Coordinator, you will be responsible for providing age-specific and developmentally appropriate quality patient-centered care, utilizing the nursing process (assessment, care planning, intervention, evaluation) in accordance with LLUMC's established Professional Practice Model.
You will work closely with a multidisciplinary team to ensure seamless coordination of patient care, prioritizing tasks and managing multiple assignments effectively. Your strong communication and interpersonal skills will enable you to relate positively and professionally with patients, families, and healthcare professionals.
In this role, you will also have the opportunity to develop your leadership skills, supervising and delegating tasks to licensed and unlicensed staff, and contributing to a work environment of caring and cooperation among a culturally diverse workforce and patient population.
Requirements- Active California Registered Nurse (RN) license required
- Basic Life Support (BLS) certification issued by the American Heart Association required
- Bachelor of Science Degree in Nursing (BSN) required
- Minimum of one year experience required
$95,000 - $115,000 per year
