Job Summary:

Key Responsibilities:

  • Supervise and mentor clinical staff to ensure high-quality patient care.
  • Develop and implement clinical programs and services to meet the needs of our patients.
  • Conduct regular client home visits to ensure quality of care and identify opportunities for improvement.
  • Manage caseloads and ensure timely and effective delivery of services.
  • Collaborate with other departments to ensure seamless care coordination.

Requirements:

  • Registered Nurse licensure in the state of practice.
  • Associates degree required.
  • 3+ years RN experience in a healthcare setting, preferably in home health or hospice.

Preferred Qualifications:

  • Medicare Skilled Nursing experience and a basic understanding of OASIS.
