About the Role:

As a Nurse Practitioner-PRN Home Risk Assessment Specialist with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks. This role requires a strong understanding of adult and geriatric care, as well as excellent communication and interpersonal skills.

Responsibilities:

Conduct home risk assessments for patients, gathering information on their medical history, current health status, and social environment.
Develop and implement individualized care plans to address identified health risks and improve patient outcomes.
Collaborate with healthcare teams, including physicians, case managers, and social workers, to ensure comprehensive care.
Provide education and support to patients and their families on health management and disease prevention.
Maintain accurate and detailed records of patient assessments and care plans.

Requirements:

Master's degree in Nursing or related field.
Current RN and NP licensure in state of practice, with prescription authority.
Board certification in a Matrix-approved specialty.
Minimum 1 year experience as a Nurse Practitioner.
Strong computer skills and familiarity with EMR software.
Excellent verbal and written communication skills.
Ability to travel and work in a variety of settings, including patients' homes, skilled facilities, and telehealth environments.
