Job Summary

We're seeking a skilled Advanced Practice Provider to join our team as a Traveling Advanced Practice Provider. As a key member of our geriatric care team, you'll provide primary, urgent, and acute medical care to patients in their homes, working collaboratively with other clinicians to deliver comprehensive care.

Responsibilities
  • Provide high-quality, patient-centered care to seniors in their homes, addressing chronic and acute medical conditions
  • Collaborate with community teams to complete care assessments and develop personalized care plans
  • Deliver quality care while maintaining company metrics on documentation, coding, HEDIS measures, and patient engagement
  • Refer patients to internal and external resources as needed, including behavioral health protocols
  • Facilitate family meetings to address complicated family dynamics and goals of care
Requirements
  • Master's degree in Nursing
  • Valid nurse practitioner or physician assistant license in the state of operations and current board certification
  • Valid driver's license, car insurance, and access to a car for patient visits
  • Expertise in geriatrics, internal medicine, palliative care, and end-of-life care
  • Strong computer skills, including Word, Excel, and PowerPoint
