Job Summary

We're seeking a skilled Advanced Practice Provider to join our team of compassionate caregivers. As a Traveling Advanced Practice Provider, you'll deliver primary, urgent, and acute medical care to patients in their homes, working collaboratively with our clinician-led geriatric care teams.

Key Responsibilities
  • Provide high-quality, patient-centered care to patients in their homes
  • Collaborate with our care teams to deliver comprehensive care, including chronic care, urgent care, and end-of-life care
  • Facilitate staff, patient, and family decision-making through educational resources and support
  • Address urgent care needs and refer to internal/external resources as necessary
  • Deliver quality care while maintaining company metrics and standards
Requirements
  • Master's Degree in Nursing
  • Valid nurse practitioner or physician assistant license in the state of operations
  • Current board certification in your field
  • Valid driver's license, car insurance, and access to a car for patient visits
