Job Title: Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at Personalized Nursing Light House INC. As a Case Manager, you will play a vital role in helping our clients navigate the complexities of the healthcare system and providing them with the support and resources they need to achieve their goals.

The ideal candidate will have a strong background in human services, excellent communication and interpersonal skills, and the ability to work effectively with individuals from diverse backgrounds. You will be responsible for conducting assessments, developing treatment plans, and coordinating services with other healthcare providers. Responsibilities:
  • Conduct comprehensive assessments of clients' needs and develop individualized treatment plans
  • Coordinate services with other healthcare providers, including medical professionals, therapists, and support groups
  • Provide education and support to clients and their families on topics such as addiction, mental health, and wellness
  • Monitor clients' progress and make adjustments to treatment plans as needed
  • Collaborate with other Case Managers and healthcare professionals to ensure seamless care coordination
Requirements:
  • Bachelor's degree in a related human services field (social work, psychology, criminal justice, etc.)
  • CAADC, CADC or Development Plan from MCBAP
  • Driver's License and a clean driving record
  • Knowledge of Microsoft Office (Word, Excel, PowerPoint)
  • Excellent communication and interpersonal skills
  • Ability to work effectively with individuals from diverse backgrounds
Preferred Qualifications:
  • Experience working with individuals with substance use disorders and/or co-occurring disorders
  • Training in trauma-related services
Work Environment:

This is a full-time position that requires working in an office setting. The ideal candidate will be able to work independently and as part of a team, and will have excellent time management and organizational skills.
