The Case Manager (CM) plays a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimal outcomes. This position requires a skilled Registered Nurse to coordinate and direct care specific to the needs of each rehabilitation patient.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Registered Nurse with an Associate Degree
- At least 1 year of RN and case management experience
