The Opportunity

We're seeking a Field Care Coordinator to join our High Risk team. In this role, you'll work outside the traditional clinic setting, visiting patients in their homes and providing telehealth support after acute inpatient events or as part of routine primary care services. Your responsibilities will include:

  • Facilitating telehealth visits with providers in patients' homes using technology to ensure complete assessments and exams.
  • Conducting standard patient assessments and documentation to facilitate comprehensive visits and ensure patients understand their care plans.
  • Clinical duties may involve providing best-in-class telehealth setup services, performing extended scope duties like physical exams, screenings, and vitals.
  • Coordinating care with patients, families, and external care teams (specialists, home health, etc.) as needed.
  • Connecting patients to community resources related to housing, transportation, food, and daily living activities.
What You'll Work On

Your key responsibilities will include:

  • Facilitating telehealth visits in patients' homes using technology.
  • Conducting patient assessments and documentation to ensure comprehensive visits.
  • Clinical duties such as telehealth setup, physical exams, and vital checks.
  • Coordinating care with patients, families, and external care teams.
  • Connecting patients to community resources.

You'll be an integral part of our High Risk Care Team, working closely with managers and colleagues to deliver exceptional patient care and outcomes.

Requirements

To succeed in this role, you'll need:

  • At least 1 year of experience in home-based healthcare services, senior living care settings, or as an Emergency Medical Technician (EMT).
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) certification preferred or ability to obtain within 6 months.
  • Strong written and verbal communication skills; Spanish speaking is a plus.
  • A proven track record of persisting through change, demonstrating a forward-thinking perspective under pressure, and consistently stepping up to address challenges.
  • A passion for building rapport and trust with patients, with excellent human connection and customer-focused care skills.

As a Field Care Coordinator, you'll enjoy a competitive compensation package, including:

$26.75-$27.75 per hour, based on your experience and qualifications.

This is a full-time role with flexible shifts generally taking place between 8:00am-5:00pm, Monday through Friday, based in Renton, Washington. You'll have the opportunity to travel throughout the Seattle area to visit patients in their homes and work from various offices as assigned.
