**Job Summary**
We are seeking an experienced Nurse Practitioner to join our team as a Hospice Care Specialist. This per diem position offers flexibility while allowing you to make a meaningful impact in the lives of our patients and their families.
**Responsibilities**
* Provide medical support to patients with complex needs
* Collaborate with our interdisciplinary team to develop personalized care plans
* Conduct regular assessments and evaluations to ensure optimal patient outcomes
* Communicate effectively with patients, families, and healthcare providers to coordinate care
* Participate in ongoing education and training to enhance your skills and knowledge
**Requirements**
* Master's degree in nursing or related field
* Active licensure as a Nurse Practitioner in the state where care is being provided
* One year of experience as a Nurse Practitioner preferred
* Valid CPR certification
* Ability to travel to all business locations
