About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our hospice care team. As a CNA, you will play a vital role in providing personalized care and support to patients and their families in their homes.

Responsibilities

  • Provide personal care services, including bathing, dressing, and grooming, as assigned
  • Assist with daily living activities, such as meal preparation and ambulation
  • Monitor and report changes in patient condition to the Case Manager
  • Collaborate with the hospice team to develop and implement individualized care plans
  • Provide emotional support and companionship to patients and their families

Qualifications

  • Current CNA certification in the state of employment
  • Minimum of six months of experience in a hospice or home health setting
  • Ability to read and follow written instructions and document care given
  • Strong communication and interpersonal skills
  • Ability to work with minimal supervision and as part of a team
