Position Overview:

The Clinic Nurse plays a crucial role in delivering comprehensive nursing care to patients within the clinic environment. Under the guidance of the Client Primary Care Provider (PCP) and the oversight of the Clinic Supervisor, the Clinic Nurse is tasked with a variety of responsibilities aimed at ensuring optimal patient outcomes.

Key Responsibilities:

  • Provide nursing care to patients as assigned, including assisting with clinic overflow, monitoring vital signs, conducting point-of-care testing, administering medications, and addressing wound care needs in alignment with established protocols.
  • Collaborate with clinic staff to manage ongoing patient care, navigate complex clinical scenarios, assist with medical procedures, and facilitate the smooth operation of clinic services.
  • Engage effectively with colleagues and stakeholders to uphold the values and mission of the organization.

Clinical Duties:

  • Execute and document prescribed procedures and care in the clinic, ensuring adherence to clinical guidelines as directed by the PCP.
  • Remain vigilant to feedback from team members, patients, and caregivers, promptly informing the RN Case Manager or other clinic personnel of any changes in patient conditions.
  • Support transitions of care between various settings, including hospitals and home health environments.
  • Dispense medications to patients during clinic visits and provide education regarding medication usage.

Home Health Responsibilities:

  • Conduct home visits as necessary, performing and documenting required procedures in accordance with clinical guidelines.
  • Provide education to patients, families, and caregivers on disease management, self-care techniques, and preventive measures.
  • Offer backup support to other nursing functions as needed.
  • Conduct pharmacogenomic testing as directed by the PCP.
  • Assist Medical Assistants with clinical procedures and manage specialty pharmacy medication orders.
  • Address medication-related inquiries and issues during clinic visits.
  • Communicate any medication incidents during team meetings and ensure appropriate follow-up actions are taken.
  • Coordinate with healthcare providers regarding medication adjustments following same-day procedures.
  • Perform pre-enrollment medication evaluations.
  • Provide coverage for various nursing roles to support patient care delivery.

Administrative Duties:

  • Maintain clinical tracking systems for screenings and vaccinations.
  • Oversee the monthly log for emergency equipment and ensure proper restocking.
  • Manage medication inventory, including the disposal of expired medications in compliance with policies.

Required Qualifications:

  • Minimum of one year of nursing experience in a clinical setting, particularly with elderly or frail populations.
  • Strong nursing knowledge and skills to effectively manage complex clinical situations.
  • Self-motivated and capable of making sound decisions in high-pressure situations.
  • Excellent clinical, organizational, and communication skills, particularly in interactions with seniors and interdisciplinary teams.
  • Ability to prioritize tasks effectively in a fast-paced environment.
  • Experience working with diverse populations and a commitment to serving vulnerable seniors.

Preferred Qualifications:

  • Bilingual in English and Spanish is preferred.

Education Requirements:

  • Associate's degree in Nursing from an accredited institution.

Licensure:

  • Unencumbered RN License.
  • BLS certification.
