Master Social Worker - End Stage Renal Disease

At Innovative Renal Care, we are seeking a highly skilled Master Social Worker to join our team. As a Master Social Worker - End Stage Renal Disease, you will play a critical role in helping patients understand their rights and responsibilities, and guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.

Responsibilities:
  • Assess patients' needs and develop personalized care plans
  • Provide education and support to patients and their families

  • Meet state regulations and licensure requirements
  • Develop and maintain strong relationships with patients and staff
Requirements:
  • Master's degree from an accredited school of social work (MSW)
