We are seeking a skilled Social Worker-Case Management to join our team at Ascension Wisconsin. As a key member of our healthcare team, you will be responsible for coordinating the overall interdisciplinary plan of care for patients, from admission to discharge.
Responsibilities- Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.
- Create a plan for care across the continuum, integrating patient/family preferences and values.
- Monitor patient care through assessments, evaluations, and/or patient records.
- Advocate for resources and removal of barriers.
- Maintain ongoing dialogue with supervisor and other health providers to ensure effective implementation of health plan.
- Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
- Licensure/Certification/Registration: One or more of the following required:
- Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job-specific experience required.
- MSW strongly preferred.
