Discover a new career path where you can make a meaningful impact in the lives of others. As a Home Health Physical Therapist with Interim HealthCare of Oakland County, you'll have the opportunity to work with patients in their own homes, providing personalized therapy and care.
Key Responsibilities:- Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
- Work as part of a home health team to develop and implement patient care plans
- Conduct assessments, document progress, and modify care plans as needed
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients and their families on therapy goals, exercises, and self-care
- Graduate of an accredited Physical Therapist Program and active PT license in Michigan
- Minimum of 2 years of physical therapy experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgment, strong interpersonal skills, dexterity, and compassion
