Position Overview:

The Clinical Nurse Navigator is a Registered Nurse whose extensive clinical expertise assists patients and their families in making informed healthcare decisions.

This role focuses on the Thoracic Oncology patient population, taking on the responsibility for planning, guiding, and assessing the delivery of care. The Navigator closely monitors patient progress throughout their treatment journey, intervening as necessary to ensure adherence to care protocols and the highest quality of care.

Working collaboratively within a multidisciplinary team, the RN Navigator facilitates timely and appropriate screening, diagnosis, therapeutic interventions, treatment, and supportive care across the healthcare spectrum.

The Navigator is accountable for coordinating all necessary resources to ensure effective patient care.

Utilizing advanced nursing knowledge, the RN Navigator identifies, implements, and enhances processes to deliver an integrative care model, while striving to achieve the highest levels of patient satisfaction.

Patient education is a critical component of this role, as the RN Navigator provides comprehensive information regarding disease processes and treatment options throughout the continuum of care.

The position also emphasizes professional development and maintaining a safe, therapeutic environment for patient care delivery.

Essential Qualifications:

Current NYS RN license/registration (or eligibility) is required.

A Bachelor's degree in Nursing or a related healthcare field is necessary, along with three (3) years of relevant RN experience, or an Associate's degree in Nursing with five (5) years of relevant RN experience.

Strong written and verbal communication skills, as well as excellent interpersonal abilities, are essential. CPR-BLS certification is required upon appointment.

Desirable Qualifications:

Preferred candidates will have three (3) years of experience as a Certified Nurse/Patient Navigator in a specialized area.
