We are currently seeking a committed Senior Care Services Coordinator to join our Senior Care Options department. This role involves assessing eligibility and ensuring that suitable services are provided to qualified members.
Key Responsibilities:
- Encourage the independent functioning of members and deliver services in the most suitable, least restrictive setting.
- Conduct initial and ongoing evaluations of members' health and functional status, and create personalized care plans.
- Build strong relationships with clients to facilitate the coordination, monitoring, and adjustment of care plans as necessary.
- Maintain regular communication with clients, including home visits, to track progress towards established objectives.
- Ensure accurate and thorough documentation of all interactions and assessments.
Qualifications:
- A genuine passion for assisting others and a commitment to making a positive impact.
- Flexibility to adapt to the evolving needs of clients and programs.
- A minimum of two years of experience in the human services sector, ideally with the elderly population.
- A Bachelor's degree in Social Work or a related field is highly preferred; current licensure is advantageous.
- Bilingual abilities are preferred.
