Job Title: Pediatric Care Coordinator

We are seeking a dedicated and detail-oriented Pediatric Care Coordinator to join our team at Connecticut Children's. As a key member of our pediatric care team, you will be responsible for coordinating the care of children and adolescents with complex medical needs.

About the Role

The successful candidate will possess excellent organizational skills, a strong understanding of pediatric healthcare systems, and a passion for working with children and families. You will work closely with our multidisciplinary team to ensure seamless care coordination and improve patient outcomes.

Key Responsibilities
  • Coordinate patient appointments, procedures, and treatments with healthcare providers, specialists, and other stakeholders.
  • Develop and maintain accurate and up-to-date patient records, including medical histories, treatment plans, and test results.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and improved patient engagement.
  • Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines.
Requirements

To be considered for this role, you must have:

  • A Bachelor's degree in Nursing (BSN) or related field.
  • A minimum of 1 year of experience in a pediatric care coordination or similar role.
  • Excellent communication and interpersonal skills.
  • Basic knowledge of electronic medical records (EMRs) and other healthcare software applications.
Salary Range:

$60,000 - $80,000 per year.
