Job Summary

We are seeking a skilled Ambulatory Clinic Nurse to join our team at Carle Foundation Hospital. As a Medicare Wellness Specialist, you will play a critical role in providing high-quality patient care and promoting health and wellness in our community.

Key Responsibilities
  • Provide comprehensive care to patients, including conducting Medicare Annual Wellness visits and assisting with clinic nursing responsibilities.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care coordination and optimal patient outcomes.
  • Stay up-to-date on the latest clinical guidelines and best practices in ambulatory care.
  • Act as a preceptor and mentor to other staff members, promoting professional growth and development.
Requirements
  • Current RN licensure in Illinois.
  • Basic Life Support (BLS) certification.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.
  • Strong clinical judgment and problem-solving skills.
