Job Summary

We are seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, skilled facilities, and through telehealth visits.

Key Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and collaborate with primary care physicians on patient care
  • Work collaboratively with case managers, social workers, and other healthcare professionals
  • Document patient information accurately and efficiently
Requirements
  • Masters degree in nursing or equivalent experience
  • Current RN and NP licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
